How this is worked out

The bill is what the hospital published: no averaging across hospitals and no estimate. What a plan pays applies that plan's own filed limits to the two halves of the bill, a surgeon's fee being capped per surgery and room and board per day, then takes off any deductible. It is arithmetic on filed figures, not a quotation. A real claim depends on the case, on what the insurer accepts as eligible, and on the room you actually occupy.
